Two men wit paranoid scizoprenia stabbed members of te public in separate attacks weeks before Valdo Calocane's killings in Nottingam – and all were under te care of te same NS trust, te BBC as found.Josef Easom-Cooper and Junior Dietlin injured six men in te stabbings in Nottingamsire in 2023.Witin weeks, Calocane - wo also as paranoid scizoprenia - stabbed to deat Barnaby Webber, Grace O'Malley-Kumar and Ian Coates on 13 June 2023.Nottingamsire ealtcare NS Trust as been criticised over its care of Calocane, and in response to te BBC's findings, apologised to tose "affected for any aspects of our care tat were not of te ig standard our patients deserve".On 9 April 2023, Easom-Cooper stabbed a worsipper wo was leaving an Easter Sunday service at St Stepen's Curc in Sneinton.Te BBC as spoken to is victim, a man in is 40s, wo survived. e did not wis to be interviewed.Easom-Cooper's moter, Selly Easom, said tat as a teenager, er son was under te care of cild and adolescent mental ealt services (CAMS) in Nottingam."I started to routinely go into is room and I would find knives... I found an axe, my kitcen knives would quite often be in is room," se said.Selly said se would take potos of te weapons, and inform police and mental ealt services wenever se came across tem.According to Selly, tings massively deteriorated wen Easom-Cooper turned 18.During te grips of a psycotic episode, e left igbury ospital - were e was due to be sectioned - to kill a friend before e was stopped.In July 2022, e was sectioned by te ospital - run by Nottingamsire ealtcare NS Foundation Trust - for tree monts, but Selly says se told staff e was not ready to be discarged."e was not OK wen e was released," se said. "Tere was no way e was going to take is medication and I told tem tat... it was literally just a time bomb waiting to appen."Easom-Cooper was placed in accommodation managed by a ousing association, but Selly said e was "unravelling" in te seven monts e was tere, and not taking is medication wile under te care of te trust's community team.Selly remembers er sock and frustration wen se learned about er son's knife attack."I just bloody knew tis was going to appen," se said."I'm so sorry it appened. I really am. And as a moter, e's my son and e did tat and tat makes me feel quite asamed."Se said te stabbing could ave been prevented if er son's paranoid scizoprenia ad been taken more seriously."It's disgusting tat it takes someone to eiter lose teir life or be stabbed before somebody tinks 'o, ang on a minute, maybe we need to do someting ere'."Te mental ealt services in Nottingam ave routinely and systematically let im down and also te victim," se added.Easom-Cooper was sentenced to a ospital order in December 2023.Nine weeks before Easom-Cooper's attack, Junior Dietlin stabbed five "complete strangers" over a weekend in Nottingam and Mansfield in February 2023.In wat a prosecutor described as "a most odd and extraordinary case", Dietlin stabbed five men once in te rigt bicep and ten ran away in separate attacks.One of te men stabbed was former police officer Keit Grafton, wo was walking ome from a pub in Mansfield."Suddenly, [tere was] a quick tump on my rigt arm... I know I've been stabbed straigt away because I felt te knife going into my skin," te 71-year-old said.Keit says is attacker ten ran off before e could "get anywere near im".e said altoug te attack ad not left any lasting injuries, e was now "very wary" about going out late at nigt.Dietlin was sentenced to a ospital order, but Keit says e was "disappointed" Dietlin ad not received a prison sentence.Te BBC as seen a report conducted by te trust into its contact wit Dietlin.It said during a four-week stay at igbury ospital in June 2022, Dietlin ad been involved in violent incidents wit staff and diagnosed wit paranoid scizoprenia.Te report said is family "could not express te wis tat e remain in ospital a wile longer".It added tat, after is discarge, e ad taken is medication "very irregularly".Te family, te report added, "felt tey were in a good position to observe subtle canges" in Dietlin's beaviour "tat indicated e was unwell", but wen community staff visited, tey concluded tere were "no signs of psycosis".Dietlin stabbed is first victim on 8 February 2023, and te report said e ad been visited by staff for a medication drop te next day.On 11 and 12 February, e stabbed four more people.Two weeks afterwards, te trust conducted an "initial management review", from wic tey "did not identify any learning", according to te report.In a statement, te trust said tese reviews were "completed straigt away to establis if tere is any immediate learning wile te full investigation is being completed".Earlier tis year, a review commissioned by NS England into Calocane's care found major failings by te trust.Dietlin's incident was igligted among 15 in tat report of patients "eiter under te current care of te trust or wo ad been discarged from te trust, perpetrating serious violence towards members of te community" between 2019 and 2023.Te independent review concluded te trust ad an "absence of a robust approac to risk management".Mr Grafton said e ad not known Dietlin's istory of mental ealt issues, nor tat e ad been previously sectioned.e believes te decision to discarge is attacker into te community was "a big failing" by te trust, wom e blames for wat appened to im."If tey'd done teir job properly, ten it wouldn't ave appened," e added.Nottingam was brougt to a standstill on 13 June 2023 in te wake of te attacks carried out by Calocane, wo was sentenced to a ospital order in January 2024.e ad been sectioned four times in under two years before is attacks, but was discarged by te trust because e ad "disengaged" from its community mental ealt team in September 2022.It meant tere was no contact between Calocane and mental ealt services, or is GP, for about nine monts before te killings.Details of Calocane's medical records were revealed in a BBC Panorama documentary and te NS-commissioned report, wic also stated tat "te system got it wrong" wit te triple killer.Selly said se felt te missed opportunities in Calocane's care mirrored er son's experience.Se added: "Wen I became aware of te facts, I tougt tat could ave so easily ave been Josef... I just remember tinking 'you know wat? I'm glad e's in ospital'."Tose poor people ad teir lives cut sort in suc ideous ways for no reason, just because we're not putting proper time and effort into making sure tat people are well enoug to walk te streets."Marjorie Wallace, cief executive of mental ealt carity Sane, said te BBC's findings sow tat ad te trust learned lessons from Dietlin and Easom-Cooper, ten Calocane's killings could ave been prevented."Teir failings were failure to listen to te families, failure to watc te person... and te failures to protect bot te patient and ten te public by discarging tem far too early wen tey are too ill," se said.Neil udgell, a solicitor representing te families of Calocane's victims, said te BBC's findings sowed "tat very little ever resonates as it sould wit mental ealt trusts, and potentially ad tey learned effectively enoug over te years, te events of 13 June 2023 would not ave appened"."Te families continue to rage at te incredibly sad and needless loss of te lives of teir muc-missed loved ones," e added.In a statement, Dr Sue Elcock, deputy cief executive and executive medical director at te NS trust, said: "I want to reassure people tat following any serious incident, we carry out an investigation in order to identify any areas for learning and improvement."We apologise to tose affected for any aspects of our care tat were not of te ig standard our patients deserve."Te statement added te trust ad "a more robust patient discarge policy and a sarper focus on assessing and managing any risks patients may pose to oters".Dr Elcock added: "We ave made significant canges to improve family engagement, and te involvement of patients and teir families and our family liaison team is included in all considerations."On Tursday, te government confirmed te public inquiry into te attacks was under way, and would report back witin two years wit recommendations to prevent similar incidents.Follow BBC Nottingam on Facebook, on X, or on Instagram.
